The orientation sensor relies on orientation with regards to gravity, not your hands. Hence, if you lie on your side with the phone in your hand, it'll switch to landscape. But your head is the same...

Electrical side You'll need capacitive sensors, but having them, You can do much more. There are capacitive ICs, but You need to cover a surface, Atmel QTouch is one option. Search it.
